Hands-on help gathering documentation, completing forms, and communicating with property managers.

All services and communications are available in both English and Spanish.

Our Step-by-Step Process

1. Referral & Intake

Referrals are received through Tri-Counties Regional Center (TCRC). Once authorized, we schedule an initial meeting to learn about your current housing situation and your vision for the future.

2. Comprehensive Evaluation

Within approximately 15 days of intake, we complete a person-centered assessment to understand your priorities and identify any potential barriers to housing. We use specialized tools to ensure the plan is truly yours:

✓ Housing Needs & Preferences Survey: Identifying your "must-haves" in a home.

✓ Important To / Important For: Balancing what makes you happy with what keeps you safe and healthy.

✓ One-Page Profile: A positive summary of your strengths and support needs to share with landlords.

✓ Financial & Safety Checklists: Reviewing budget and accessibility needs.

3. Housing Search & Applications

Using our secure, continuously updated housing database, we identify options that align with your specific criteria. Support includes:

  • Organizing required documentation and submitting applications.

  • Navigating and monitoring housing waitlists.

  • Direct follow-up with landlords and housing authorities.

4. Securing Housing Placement

Once a unit is identified, our team supports you through the final steps of move-in:

  • Reviewing housing options and selecting the best unit.

  • Helping you understand and sign lease terms.

  • Preparing for the transition to your new home.

5. Transition to Stabilization

After placement, you may transition to Housing Stabilization services to help you maintain your new home and address ongoing tenancy needs.

Expected Outcomes

Our goal is to empower you toward long-term independence.

Success for us looks like:

✓ Securing stable housing, typically within 3–6 months.

✓ Maintaining that housing for at least 12 months.

✓ Strengthening your financial management and budgeting skills.

✓ Increasing your confidence in navigating housing systems on your own.

Who We Serve

Housing Coordination is available to individuals of all ages with developmental, intellectual, or cognitive disabilities who receive services through TCRC and are willing to engage in the housing process.
